Everi EHR Bridge Extension: Privacy Notice

This notice covers the Everi EHR Bridge extension for the browser, provided by Everi Labs Inc. The extension is for clinics that use the Everi application together with a supported electronic health record (“EHR”) system. For other activities, please see Everi Labs’ general Privacy Notice .

1. What the extension does

The extension lets clinicians move patient information between the Everi application and the clinic’s EHR system in the clinician’s browser. It runs only on the Everi application and on supported EHR web pages.

2. What information it handles

Depending on the transfer, this may include:

• Patient demographic and identifying information;
• Clinical notes;
• Prescription and diagnosis information;
• Insurance information;
• Files the clinician chooses to transfer.

3. How the information is handled

Transfers take place within the clinician’s browser, directly between the Everi application and the EHR. The extension does not send patient information to Everi Labs’ servers or to any third party, and does not keep patient information after a transfer.

Once the information is in the Everi application, your clinic’s agreement with Everi Labs applies to it. Once it is in the EHR, the EHR vendor’s terms apply to it.

4. What the extension stores

The extension stores its settings on the clinician’s device, such as which EHR it connects to.

5. What the information is used for

The extension uses any information collected only to perform the transfer. Our use of this information complies with the Chrome Web Store User Data Policy, including its Limited Use requirements.

If we make changes to this Notice, we will notify you on our Website. You can see when this Notice was last updated by checking the “last updated” date displayed at the top of this Notice. Therefore, please review this Notice from time to time so that you are aware when any changes are made to this Notice. Significant changes to how we collect or process your personal data will be notified to you via email.
